What are the conditions related to emphysema?

1. Chronic Bronchitis

  • Often coexists with emphysema in COPD patients.

  • Characterized by a persistent productive cough for ≥3 months in 2 consecutive years.

  • Main difference: more mucus production and bronchial inflammation vs. alveolar destruction in emphysema.

2. Alpha-1 Antitrypsin Deficiency

  • Genetic disorder (autosomal recessive) causing early-onset panacinar emphysema, especially in lower lobes.

  • Alpha-1 antitrypsin protects lung tissue from proteolytic enzymes (like elastase).

  • Deficiency → unchecked tissue destruction.

3. Recurrent Respiratory Infections

  • Damaged airways and impaired mucociliary clearance increase susceptibility.

  • Common infections: Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis.

4. Pulmonary Hypertension

  • Chronic hypoxia → pulmonary vasoconstriction → increased pulmonary arterial pressure.

  • Leads to right-sided heart strain.

5. Cor Pulmonale (Right Heart Failure)

  • A complication of severe emphysema and chronic hypoxia.

  • Right ventricle hypertrophy and failure due to long-standing pulmonary hypertension.

  • Symptoms: edema, hepatomegaly, raised JVP.

6. Spontaneous Pneumothorax

  • Seen especially in paraseptal (distal acinar) emphysema.

  • Rupture of subpleural blebs or bullae can lead to air leaking into the pleural space → lung collapse.

  • Common in young, tall, thin males or severe COPD patients.

7. Respiratory Acidosis

  • As emphysema progresses → CO₂ retention (hypercapnia) → respiratory acidosis.

  • Symptoms: headache, drowsiness, confusion.

8. Osteoporosis

  • Chronic corticosteroid use and chronic inflammation may contribute.

  • Associated with reduced physical activity and systemic effects of COPD.

9. Lung Cancer

  • Common in patients with emphysema anda longg-term smoking history.

  • Squamous cell carcinoma and small cell carcinoma are most common.

10. Anxiety and Depression

  • High prevalence in COPD/emphysema patients due to chronic breathlessness and reduced quality of life.
